ASO Author Reflections: Clinical Significance of Primary Tumor Side in Peritoneal Metastatic Colon Cancer

Colon cancers are classified as right- or left-sided based on the location of the primary tumor relative to the splenic flexure. It is known that colon adenocarcinomas arising from the right side are clinically and biologically different from those arising from the left side in terms of presentation, pathologic characteristics, molecular aberrations, and outcomes.1 A right-sided primary tumor has been shown to be prognostic of worse disease-free (DFS) and overall survival (OS) in patients with systemic metastasis treated with palliative chemotherapy, and in patients with regional liver metastasis treated with curative-intent surgery.2 Patients with metastasis to the peritoneum represent a small subset of colon cancer patients, but they have a particularly poor prognosis.3 The impact of primary tumor side on patients with peritoneal metastatic colon cancer treated with cytoreduction with or without hyperthermic intraperitoneal chemotherapy (HIPEC) has not been well characterized. …
