High-flow nasal cannula oxygen therapy (HFNC) as long been considered a safe treatment, hence the term “gentle CPAP” partly describing its mechanism. A 2-month-old infant was admitted to PICU having developed a massive right lung (RL) overinflation while on HFNC (2 L/kg/min, FiO2 0.6) over a 3-day course in the pediatric ward. On admission, the infant appeared polypneic and distressed. His clinical history reported normal birth and a bronchiolitis episode 3 weeks before. Both the HFNC device and the nasal cannula size were appropriate, as was the humidification setting. …
