A 40-year-old female with newly diagnosed extensive ulcerative colitis (UC) was evaluated for acute severe UC (ASUC), with upwards of fifteen loose bloody bowel movements, nocturnal stools, and a thirty-pound weight loss. Infectious evaluation, including stool studies was negative. Previously in good health, she did not have a history of any gastrointestinal symptoms. She underwent an outpatient colonoscopy that showed Mayo 3 colitis throughout the colon with a normal terminal ileum. Biopsies showed severe chronic active colitis without cytomegalovirus inclusions. She was started on prednisone 30 mg daily for a month with no improvement in symptoms prior to presentation at our IBD Center. …
