I read with interest the randomized clinical trial by Li et al. [
1] that emphasized the importance of radiation therapy (RT) in locally advanced rectal cancer (LARC) management. During the past 30 years, major advances have helped to substantially improve the prognosis of LARC patients: (i) the use of preoperative chemoRT increased locoregional control; (ii) the advent of intensity modulated RT technique allowed a better dose distribution, assuring a better adjacent normal tissues sparing; (iii) the use of magnetic resonance imaging (MRI) assured a superior definition of local extension, especially into mesorectal fat. Consequently, the vast majority of LARC patients have a good outcome with a minimization of RT side effects. …
