Why can flu be so deadly? An unusual case of cardiogenic shock

A 66-year-old man was admitted to the emergency department of a district general hospital with a 2-day history of asthenia, fever and mild chest discomfort. His past medical history was unremarkable. Blood pressure was 105/66 mmHg with a heart rate of 95 bpm and oxygen saturation of 98% in ambient air. Physical examination revealed no signs of pulmonary or peripheral congestion with normal peripheral perfusion. Abdomen and cardiac evaluations were within limits. Inflammatory markers (CRP 85 mg/dL) and cardiac troponin I (112.1 pg/mL) were elevated. Transthoracic echocardiography (TTE) showed thickened hyperreflective interventricular septum, normal biventricular function and mild pericardial effusion. …
