Portal vein tumor thrombosis (PVTT) has been identified in 10–40% of hepatocellular carcinoma (HCC) patients at initial visit. According to the Barcelona Clinic Liver Cancer (BCLC) classification, portal invasion is categorized as an advanced stage with a median survival of 7.9 months. Several modalities have been attempted to increase survival, but overall survival rarely exceeded 1 year, except for surgical resection, with median survival ranging from 9 to 33 months.1 Although the results of upfront surgery seem superior, we should be reminded of the associated high selection bias. …
