Bile duct invasion (BDI) is a relatively rare presentation in hepatocellular carcinoma (HCC) with an incidence of 1.2–9.0% reported in the literature mainly based on operative findings and autopsies. The presence of bile duct invasion in HCC patients may indicate a poorer prognosis than those without bile duct invasion. Furthermore, HCC with hilar bile duct tumor thrombus may mimic hilar cholangiocarcinoma (Klatskin’s tumor) on enhanced computed tomography (CT) and cause diagnostic difficulties for clinicians.
