Given that emboli are responsible for the majority of non-lacunar ischemic strokes, a standardized evaluation is recommended to determine major cardiac and arterial sources.1 Major cardiac sources (e.g., atrial fibrillation and left ventricular thrombus) are identified by electrocardiogram (ECG) and transthoracic echocardiography. Imaging of the carotid and cerebral arteries, such as computed tomography (CT) and magnetic resonance imaging (MRI), can identify common atherothrombotic embolic sources. When embolic sources are not identified by the standard approach, non-lacunar strokes are deemed embolic stroke of undetermined sources (ESUS), which may merit from further evaluation.1
