Until recently, residents, faculty, nurses, medical assistants and clerks in our urban academic clinic worked in silos. Far too often, part-time resident or faculty primary care providers (PCPs) were unavailable when their patients needed to be seen and patients saw different providers and staff each time they came to clinic. Our 118 staff and providers—who often did not know each other’s names—had no structured systems for communication around ongoing patient needs. Confusion about roles and responsibilities led to duplication of work, lack of care coordination, and unnecessary delays in care.
