Encephalocele is increasingly recognized as a treatable cause of drug-resistant temporal lobe epilepsy (TLE). The spectrum of electroclinical manifestations includes focal seizures with or without impaired awareness and focal to bilateral tonic–clonic seizures [

2]. The incidence of encephalocele in TLE patients is still unknown. Current literature reports that many cases are identified through retrospective review of MRI scans previously reported as “negative.” There is no shared consensus on the encephaloceles therapeutic management. However, the efficacy of surgical treatment regardless of the choice of conservative approach by lesionectomy or extensive resection by anterior temporal lobectomy was reported [
