Chest pain is common in patients with hypertrophic cardiomyopathy (HCM)1, although it is very often not due to epicardial coronary disease2. Both outflow tract obstruction and microvascular dysfunction have been suggested as potential mechanisms of angina. Small intramural coronary artery dysplasia (SICAD) is ubiquitous in this condition3, and repetitive microvascular ischemia could be a promoter of interstitial fibrosis—which is present in almost three quarters of HCM patients to varying degrees. …
