An 81-year-old woman with severe aortic stenosis was referred to our hospital for transcatheter aortic valve replacement (TAVR) via transapical approach. Although the patient had a history of coronary artery disease with multiple stents, the echocardiogram did not show left ventricular (LV) wall motion abnormalities or thinning. TAVR was performed through a left anterolateral mini-thoracotomy. An Ascendra + introducer sheath (24-Fr) was inserted through double purse string sutures at the LV apex.
