In the newborn nursery, a 1-day-old girl was noted to have a diastolic murmur at the base of the heart. A transthoracic echocardiogram showed an unusual form of truncus arteriosus in which the ventricular septal defect (VSD) was remote, and the truncal valve arose exclusively from the right ventricle.
