Transthyretin cardiac amyloid and aortic stenosis in the elderly, the role of nuclear imaging

Cardiac amyloidosis is an under recognised cause of heart failure in the elderly [1]. It is predominantly related to myocardial deposition of wild-type transthyretin protein (aTTR) causing a restrictive cardiomyopathy and it is associated with poor prognosis [2]. Radionuclide bone scan is able to detect transthyretin myocardial deposition. Moderate and intense captation by the myocardium has been demonstrated diagnostic without need of myocardial biopsy [3]. This tool has given us the possibility to expand the diagnosis of transthyretin cardiac amyloidosis. Data from post-mortem studies in unselected subjects indicate a prevalence of cardiac amyloidosis of 22–25% in subjects older than 80 years of age [4]. …
