Hourglass appearance of the gallbladder can be observed on multiple imaging modalities in segmental adenomyomatosis (Fig. 1A, B) [1, 2]. Adenomyomatosis of the gallbladder is a benign hyperplasia of the gallbladder mucosa found in up to 8.7% of cholecystectomy specimens [3, 4], and is classified into segmental, localized fundal, and diffuse types. The segmental type is most common [5], involving annular mural thickening that results in stricture of the gallbladder lumen, producing the characteristic hourglass appearance (Fig. 2) [1]. Although usually asymptomatic and traditionally considered a ‘don’t touch’ lesion, segmental adenomyomatosis has been associated with a higher prevalence of gallbladder cancer [3], and surgery may be considered in some cases [6].
