Dopamine agonists are gaining acceptance not only for the treatment of advanced Parkinson’s disease but also for the initial treatment of early-stage disease. This represents a change from earlier concepts on the management of Parkinson’s disease, which were mainly based on the use of levodopa coupled with dopa-decarboxylase inhibitors. However, the daily ‘peak’ and ‘trough’ blood concentrations of the multiple-dose oral dopamine agonists result in pulsatile stimulation of dopamine receptors, which may contribute to the pathogenesis of motor complications. …
