The challenge of prognostication in palliative radiotherapy: the way forward is shared decision-making

We were very interested to read the useful update by Hui et al. on the challenging issue of prognostication.1 We fully agree on the need to improve the science and art of prognostication. The literature confirms that the increasing tendency to treat advanced cancer patients up until the very end of life, referring them to Palliative Care (PC) services when it is all but too late, has been identified as an index of poor Quality of Care (QoC).1 We wanted to reiterate the importance of the topic broached by Hui et al. and to focus attention on the somewhat controversial setting of palliative radiotherapy (PRT). …
