Nuclear medicine (NM) as an independent medical specialty for more than 25 years (recognised by the European Directives 1988) continues to progress and impact on clinical management of patients with a wide variety of diseases. The particular characteristics of the radiopharmaceuticals have made it possible to open up new avenues in the diagnosis, staging, restaging and evaluation of response to therapy. In addition, they made it possible to systemically treat some diseases with the same molecule used for diagnosis, but labelled with a different radionuclide. This approach, nowadays called theranostics, may find its beginnings in the early 1930s when 131I was made available to evaluate the thyroid function by imaging “cold” (non-functioning) and “hot” (functioning) nodules and to treat thyroid cancer metastases as well as hyperthyroidism later on. This continues to be the current practice with small procedural changes. …
