A 77-year-old male with a history of diabetes mellitus and myocardial ischemia underwent total joint replacement on his right knee for osteoarthritis. During the next six years, surgical debridement and antibiotic courses were repeated against prolonged prosthetic joint infection until the prosthesis was eventually removed surgically. The synovial fluid taken on the day of joint removal revealed methicillin-resistant coagulase-negative staphylococcus (resistant to levofloxacin and trimethoprim-sulfamethoxazole) and unidentified gram-negative bacilli. Due to the concern of residual prosthetic materials and patient’s wish, life-long chronic suppression with oral minocycline (100 mg bid) and levofloxacin was started. …
