A 79-year-old woman presented with sudden onset of diplopia. Neurological examination revealed conjugate right gaze palsy along with impaired adduction in the right eye and abduction with nystagmus in the left eye, right peripheral facial paralysis, and left-side hemihypesthesia. Brain CT scan and MRI showed right paramedian pontine tegmental hemorrhage (Fig. 1a–d).×
