The concept of “Population Health” is the understanding of the factors that modulate health outcomes of individuals living in a specific geographic area. This concept includes defining those distinguishing features amongst individuals residing in the same area that result in different outcomes. The data collected by the population health approach should lead to policy, regulation, and systems level change focusing on the economic, environmental, and social components that can lead to meaningful change in health outcomes for communities (
