We present two cases of myocardial ischemia diagnosed incidentally at the time of oncologic 18F-fluorodeoxyglucose positron emission tomography (FDG PET) imaging. These cases highlight the potential of FDG PET cardiac metabolic imaging to detect acute or recent myocardial ischemia. When reporting oncological FDG PET/CT staging studies, it is important to differentiate focal and intense myocardial uptake in a vascular distribution from physiologic uptake, which is usually less intense, more heterogeneous and does not conform to a coronary territory. …
