To the Editor: A 15-mo-old girl with preexisting global developmental delay presented with lethargy, poor feeding, abnormal involuntary movements and rapid breathing for 2 d. There was no prodromal illness, fever, vomiting, seizures, or toxin or drug exposure. She was first born to a non-consanguineous couple and had an uneventful perinatal period. She was exclusively breast-fed, and the family followed a strict vegetarian diet. …
