Renal Cell Carcinoma Metastatic to the Gallbladder

A 66-year-old female was referred for an enlarging gallbladder mass. Whole-body CT scan demonstrated an approximately 3-cm gallbladder soft tissue mass without other visible abnormalities (Fig. 1). The patient had a history of left nephrectomy for T2N0M0 clear cell carcinoma 2 years prior and underwent laparoscopic cholecystectomy with en bloc partial resection of segment IVb. Surgical pathology revealed polygonal cells with clear cytoplasm and round nuclei staining positively for CD10, vimentin, and PAX8 and negatively for S-100, MART-1, and desmin, consistent with the diagnosis of metastatic renal cell carcinoma (Figs. 2 and 3).
