Colorectal cancer [CRC] is the third most commonly diagnosed and cause of cancer related deaths in both genders in the USA [1].The incidence varies by age and is most common among adults >50 years [2]. Brain metastasis is rare with an incidence of 1–4% [3].Most brain metastases develop during the course of disease while the presence at initial presentation is rare. RAS mutation is associated with metastasis in CRC [4]. We present a 36-year-old male with brain metastasis as the initial presentation of a RAS unmutated rectal adenocarcinoma with microsatellite instability [MSI] and multiple synchronous metastases. …
