Infective Endoshuntitis in a Patient with Non-cirrhotic Portal Hypertension: Successful Medical Management

Prosthetic mesocaval grafts for the management of portal hypertension were introduced in the early 1960s. Complications such as shunt thrombosis, malfunction, non-function, and infection result in significant morbidity and mortality [1]. Shunt thrombosis occurs especially if surgery is done in the emergency setting or in patients with an underlying hypercoagulable state. Infection of the prosthetic graft is rare; it usually occurs in the early post-operative period and necessitates surgical shunt revision. We present a patient with non-cirrhotic portal hypertension with underlying hypercoagulable state who developed late infection of thrombosed mesocaval shunt that was successfully treated with prolonged antibiotic therapy without surgical revision. …
