Local excision appears to be the mainstay of treatment to obtain adequate local control for perianal giant condylomas. Radical reconstructive surgery should be reserved for patients with malignant transformation, recurrence, or anal sphincter involvement. However, morbidity in patients undergoing extensive excision is high. Adjunct radiotherapy, photodynamic therapy, and imiquimod have been proposed in advanced or recurrent lesions. A 58-year-old human immunodeficiency virus–negative man was successfully treated for giant perianal condyloma using imiquimod 5% without surgery. Complete resolution of the condition was achieved after 2 months of topical treatment (Figs. 1, 2, 3, and 4).×
