Orale Manifestation einer miliaren Tuberkulose

Fallbericht

Es wird über den klinischen Verlauf eines 40-jährigen Patienten mit der oralen Manifestation einer miliaren Tuberkuloseinfektion berichtet. Neben Mundschleimhaut und Lunge waren Kolon und Ileum ebenfalls befallen. Diese oralen Schleimhautläsionen können einer malignen Erkrankung ähneln und sind klinisch schwierig zu differenzieren, insbesondere da die Tuberkulose in den industrialisierten Ländern selten geworden ist.

Diskussion

Orale Läsionen und auffallende pulmonale Beschwerden sollten ein Alarmsignal für eine bestehende Tuberkulose sein. Patienten mit derartigen Beschwerden sind einer geeigneten Diagnostik zu unterziehen. Die wichtigsten Diagnostikschritte sind eine Röntgenaufnahme des Thorax und Biopsie aus dem verdächtigen Bereich der Mundhöhle für histologische Untersuchungen. Parallel sollte mit der Koch-Diagnostik, dem Nachweis säurefester Stäbchen in Sputum, Magensaft und Urin, begonnen und Kulturen angelegt werden. Bei negativem Ergebnis kann ggf. im Rahmen einer PCR die DNA von Mycobacterium tuberculosis nachgewiesen werden.
