Acute kidney injury as a direct consequence of hemophagocytic lymphohistiocytosis [HLH]. CD68 positivity signifies histiocytic infiltrate. Few histiocytes also showed lymphocytic phagocytosis.

To find the cause of HLH, all infectious etiologies (e.g. typhus, malaria, dengue), malignancy and genetic sequencing to rule out primary HLH
 
3.
Pulse steroids followed by oral prednisolone in view of predominant histiocytic infiltrate with tubulointerstitial nephritis
