We have observed with a mixture of amazement and disbelief recent events concerning proposals for the next phase in the development of endoscopic surgery. It appears that the endoscopic surgical community on both sides of the Atlantic has given tacit acceptance to revolutionary, if not outlandish, concepts. Moreover, to our knowledge, not a single dissenting opinion against these proposals has been expressed in scientific journals nor any objective assessment of the likely clinical benefit from such interventions. Hopefully, we all agree that clinical benefit must remain the raison d’être of all surgical research and development (R&D). …
