Unlocking Precision in Radioembolization: Navigating the Future of Holmium-166 Radioembolization Mapping and Lung Shunt Study by Implementing Scout Dosimetry

In this issue of Cardiovascular Interventional Radiology Journal, Wagemans et al. [1] studied the correlation of estimated lung mean dose (eLMD) based on Technetium-99 m labeled MAA (99mTc-MAA) and scout dose of Holmium-166 (166Ho) with post-treatment 166Ho-SPECT/CT scan in 37 patients with unresectable liver metastases with a hybrid SPECT/CT system. There was a significant moderate positive correlation between eLMD based on scout dose of 166Ho and post-treatment 166Ho (r =  + 0.43, p = 0.01). However, not only no correlation existed between eLMD based on 99mTc-MAA and post-treatment 166Ho (p = 0.90), but also eLMD based on 99mTc-MAA overestimated the eLMD post-treatment 166Ho. The authors concluded that scout dose of 166Ho is superior to 99mTc-MAA in predicting eLMD [1]. …
