A 15-year-old girl presented with a short history of a nonspecific pyrexial illness. Staphylococcus aureus was grown on blood culture, but the primary focus was not immediately identified. A pericardial effusion and a suspicious lesion on her mitral valve were found on day 7 of the illness (day 7, Fig. 1a). The effusion caused tamponade when cardiac arrest followed acute severe mitral regurgitation, and multiple large vegetations were discovered by echocardiography during resuscitation (day 8, Fig. 1b, c).
