Mechanical assistance in the surgical treatment of massive abdominal pannus

Abstract

A 32 year-old male patient was effectively immobilized secondary to morbid obesity. He underwent a gastric bypass which reduced his weight from 647 lbs to 376 lbs over 12 months. A massive abdominal panniculus persisted limiting his ability to pursue an effective exercise program and maintain appropriate hygiene. The patient was operated upon with the aid of Steinman pins to pierce the dependant portion of the pannus and an orthopaedic suspension device to facilitate its elevation. Operative excision of the 19.93 kg pannus proceeded in a rapid fashion with minimal difficulties in pannus manipulation. The technique of excision, its benefits and the case history are reviewed.
