Ten things ICU specialists need to know about new valvular procedures in interventional cardiology

Every day, intensivists around the world are confronted with new and rapidly growing numbers of cardiac devices as part of an ever-evolving approach to treat heart diseases. Particularly noteworthy in this context are: one, transcatheter aortic valve replacement (TAVR); two, trans-septal procedures including transcatheter mitral valve repair (TMVR); and three, transcatheter tricuspid valve repair (TTVR). Different risk profiles of these procedures call for constant vigilance in terms of recognizing and reporting complications. There is a trend towards avoiding ICU admission for some of these patients, particularly for TAVR procedures, and the intensivist is likely to encounter complications in patients treated in the coronary care unit or onwards at various times post procedure. In this article, we outline ten important complications ICU specialists need to know related to these new procedures. Moreover, the extended heart team, which also includes the intensivist, can be an excellent organizational equivalent for planning of valvular procedures and identifying patients at highest risk for these procedure-specific complications. …
