Sir: Accumulation of the biguanide antihyperglycemic agent metformin has been linked to lactic acidosis, a complication associated with a high mortality rate in critical illness. However, there is mounting evidence that metformin accumulation has no adverse prognostic significance per se and may even be protective in severe pathologies [
1]. Here the complete recovery of an elderly woman who presented with prolonged hypoglycemic coma after sulfonylurea intoxication and metformin accumulation provides a new argument for a possible beneficial effect. …
