To the Editor: In early studies, the use of continuous subcutaneous insulin infusion (CSII) was associated with an increased rate of diabetic ketoacidosis, caused mainly by infusion system malfunction [1‐3]. However, these findings were not confirmed in most subsequent studies [4, 5]. It has been suggested that technological improvements and/or patient and physician vigilance has lowered this risk. Nevertheless, the reliability of pumps has not been re-evaluated during the last two decades. …
