In 2017, the US Department of Veterans Affairs (VA) Electronic Health Record Modernization (EHRM) program was launched to replace the VA-developed Veterans Health Information Systems and Technology Architecture (VistA) and Computerized Patient Record System (CPRS) with the commercially available Cerner Millennium electronic health record (EHR).
1 EHRM has been complicated by leadership changes and deployment delays. After the initial deployment in 2020, several problems were documented including training deficiencies, planning weaknesses, cost overruns, and most alarmingly direct harm to Veterans.
