Dysautonomia and response to guanfacine in individuals with an SCN9A variant

I read with interest the recent article by De Cassia Collaço et al., who reported two unrelated individuals with symptoms of anxiety and “dysautonomia” and experiencing a symptomatic response to 1 mg/day guanfacine [
6]. The authors postulated that this response to what they considered a low dose of guanfacine was possibly due to a “particular genetic deficit” resulting in heightened sensitivity to the drug. Whole exome sequencing (WES) revealed a variant in the gene
SCN9A (which encodes for the voltage-gated sodium channel Nav1.7) in both subjects. The authors concluded that this variant was responsible for the response to low-dose guanfacine and causally related to anxiety and “dysautonomia” in these individuals. …
