Recurrence after curative resection of hepatocellular carcinoma (HCC) is common. Although recurrence of HCC is most likely to occur at an intrahepatic site due to residual disease or de novo lesions,
1 extrahepatic recurrence of HCC can indicate a more aggressive tumor phenotype and can be associated with a worse prognosis.
2 In addition, treatment of extrahepatic recurrence can be more challenging. Despite this, well-selected patients with extrahepatic recurrence may benefit from repeat resection, locoregional therapy, radiotherapy, and systematic therapy, including molecular-targeted agents and immune checkpoint inhibitors.

4 As such, evaluation and prediction of patients with certain risk factors for extrahepatic recurrence is of importance in postoperative surveillance, as well as consideration of adjuvant treatments. …
