Skinner-Robertson et al. recently reported in this journal on the enantiomer-specific pharmacokinetics of tramadol extended-release tablets in a cohort of 15 elderly subjects with mild renal insufficiency compared with 20 young adult subjects [1]. We value this compound-specific observation, but also would like to highlight its relevance as a model to assess the impact of organ-specific physiological decline in function on age-specific pharmacokinetics and stress the similarities with observations in young infants. …
