Achromobacter xylosoxidans Pneumonia in a Young Child with Chronic Granulomatous Disease—a Case-Based Review

Chronic granulomatous disease (CGD), an important inborn error of immunity (IEI) affecting phagocytes, is characterized by a defect in the oxidative burst. Chronic or recurrent infections, predominantly due to catalase-positive bacteria and fungi, are the most important clinical phenotype. However, infections due to Achromobacter sp., a group of catalase-positive bacteria, have rarely been reported in CGD. We report a young male child with X-linked CGD who developed persistent pneumonia due to Achromobacter xylosoxidans. Reports of Achromobacter sp. infections in patients with an IEI are sparse. Our report also reviews the characteristics and outcomes of Achromobacter sp. infections in patients with an IEI. In addition to cystic fibrosis, secondary immunodeficiency, and healthcare-associated infections, it seems prudent to consider an IEI whenever A. xylosoxidans is isolated. Timely diagnosis and treatment of A. xylosoxidans infections and the underlying condition are crucial for best therapeutic outcomes. …
