When I was a junior resident, one of my attending staff anesthesiologists gave me the following advice: “When you’re intubating someone, hold your breath; if you haven’t got the tube in by the time you have to breathe, it’s probably a good idea to back off and mask ventilate the patient.” Like all aphorisms, there was some truth to this, but it was admittedly before apneic oxygenation started gaining favour. …
