The delivery of critical care medicine (CCM) services in the USA is expensive [

2], exceeding US$80 billion in 2005 [
2]. However, despite the CCM cost magnitude, CCM costs are never reported by any US governmental agency; instead, they are calculated periodically, since the 1980s, by various groups of investigators [
