Sir: We found the recent contribution by Barret and Bion [
1] very interesting; however, we feel obliged to comment on some misleading information reported about the training of critical care specialists in Spain. Training for all medical specialties in Spain is regulated by law (Real Decreto 127/84 por la que se regula la formación médica especializada y la obtención de título de médico especialista, B.O.E. 26/1984 del 31 de Enero de 1984, p. 2524); the current legislation does not permit either subspecialization or supraspecialization. Indeed, the only specialty recognized for the care of critically ill patients in this legal framework is “intensive care medicine.” Residents in this program undergo 5 years’ training in all aspects of care for the critically ill patient. …
