In this issue of the Journal, van den Brandt [
1] examines the relationship of smoking before and after menopause with the risk of postmenopausal breast cancer in a case-cohort investigation conducted within the Netherlands Cohort Study (NLCS). He reports that, when smoking during the pre- and postmenopausal periods was mutually adjusted for, postmenopausal breast cancer risk was positively associated with premenopausal smoking (HR
per 20pack-years = 1.35, 95% CI 1.10–1.65), but inversely associated with postmenopausal smoking (HR
per 20pack-years = 0.47, 95% CI 0.28–0.80), both in a dose-dependent manner. …
