This communication is in response to the views expressed by Agarwal et al. in their article published in the August 2009 issue [
1]. We agree with the authors regarding the need for a different strategy for breast cancer screening in India. Some Indian authors have also documented the need for diverting available resources to public health education [
2]. The incidence of breast cancer in India is said to be on the rise, but studies have not confirmed this. In 1991 the incidence was estimated to be 19.3 cases per 100,000 population [
3]. An increased incidence was documented only in certain urban cancer registries like Delhi and Chennai; other urban and rural registries have not reported an equivalent rise [
