To the Editor—In our study, we examined long-term outcomes of patients originally randomized to receive topical nitroglycerin or lateral internal sphincterotomy (LIS). Although the patients were originally enrolled in a randomized, controlled trial, at the completion of the six-month follow-up period, patients were treated at the discretion of their surgeons and/or family physicians. As such, the long-term study is neither a randomized, controlled trial nor a cross-over study but a cohort study that examines outcomes, including the subsequent need for further treatment, with two different initial treatment strategies. …
