We appreciate Losanoff and Millis’s letter proposing that the preperitoneal tunneling method for the placement of permanent peritoneal dialysis catheters (PDC) be accorded greater consideration because it can prevent some of the known complications of permanent PDC, such as catheter exit site leaks and catheter migration. They suggest that our method has the potential risk that injuries to the epigastric vessels will be inflicted during both PDC insertion and removal, as well as a risk for intramural leaks in the preperitoneal, intramuscular, and subcutaneous planes around the catheter. They also ask about the morbidity associated with the surgical removal of such catheters when the distal cuff is in the preperitoneal space. …
