Small cell lung carcinoma invading the pulmonary vein and left atrium as imaged by PET/CT

A 78-year-old female presented with chest pain and shortness of breath. Chest radiograph showed a right suprahilar mass. Bronchoscopic biopsy revealed small cell carcinoma. PET/CT using fluorodeoxyglucose showed intense glycolytic activity in the right suprahilar mass. It has a “flower” configuration, complete with four petals and a stem. The “petals” reflected the lobulated nature of the mass, with regions of internal necrosis. The “stem” corresponded to the hypermetabolic tumor thrombus in the pulmonary vein extending into the left atrium. No adenopathy or distant metastasis was identified. …
