Roux-en-Y gastric bypass (RYGB) is the most common bariatric procedure for morbid obesity [1]. In these patients, access to the major papilla is a real challenge. Many alternative approaches, including enteroscopy-assisted [2, 3] or surgery-assisted [4, 5] ERCP to manage pancreaticobiliary disorders in patients with RYGB, have been reported. Actually, these techniques are associated with a low success rate or high risk of complications. Recently, EUS-directed gastro-gastrostomy ERCP (EDGE) was developed as an effective mini-invasive treatment for patients with RYGB [68].
